●Successfully pass a drug screen, criminal background check, employment physical and TB test prior to beginning employment.
●Provide documentation proving employment eligibility, such as a Passport, or a driver's license and Social Security card or Birth Certificate.
●Provide immunization records, specifically MMR, Varicella and Hepatitis B
●Provide bank account information for payroll direct deposit
●Provide current professional licensure (if required)
●Provide a copy of CPR certification (if applicable)
