Leave of Absence

Unpaid leave for education, injury or other personal reasons. Job not guaranteed at end of leave.

Military Leave

Immediate

Leave of absence granted for military leave will be without pay.  An employee may use PPL time while absent for military service or training.

Family & medical leave act (FMLA)

MCHHS & You

After 1 year and 1250 hours

Up to twelve weeks leave for personal or family medical condition or for well child care immediately following birth or adoption of baby. Job guaranteed and insurance continued during leave.

1st of month following 30th day of employ. Must avg. 87 hours/month  MCHHS offers vision coverage with 3 options; Employee, Employee +1 or Family.  Please see vision Care Direct rate sheet for details.

Dependent Life Insurance

After 6 months of employ and avg. 87 hrs/month

Coverage equals one to five times annual salary based on age and salary, rounded to next $1000 dollars. Benefit doubles for accidental death. Additional benefits if death occurs while protected by a seat belt or airbag. Accelerated benefits are available for terminally ill employees.

 

$10,000 coverage for spouse and dependents under age 21 or a student age 25.

Long Term Disability

MCHHS

After 6 months of employ and avg. 87 hrs/month

Income protection when off due to long term illness (60 days or more) of up to 60% of your wages.

Supplemental Life

Immediate

Guaranteed Issue: The employee may purchase up to $150,000 on self; $50,000 on spouse and $20,000 on children.
