When you are scheduled for an outpatient procedure or an acute admission, you will check in with Patient Registration/Admission personnel, before your appointment. This includes any Lab, X-Ray, Surgical, Physical Therapy, Occupational Therapy, and Speech Therapy.

Patient Instructions
Please report to one of the three desks in the hallway off the front lobby to sign in and give pertinent information. You will need to bring your current insurance cards and any information pertaining to your visit. (Accident info, date, time, contact persons, telephone numbers, etc.)
If we are with another patient, please have a seat and we will be with you as soon as we are able. |

We may call you in advance if you are scheduled for a procedure to pre-admit you. This saves you time. If you are an obstetrical patient and will be having your baby here, it is helpful if you come in to pre-admit ahead a time, about 3-4 weeks prior to delivery date.
If you are an emergency patient or a patient needing assistance after hours, please go to the emergency room on the west side of the hospital.
