Our Hospice Team & Services


Your care will be provided by our team consisting of medical professionals, including nurses, home health aides, physicians, spiritual care, social workers, and trained volunteers. Our staff has many years of hospice care experience. And, along with caring for patients, these individuals make home visits, provide respite for caregivers, and coordinate community services by listening and responding to the needs of the patient and family.

The patient’s primary physician continues to oversee daily care whether the patient is at home, the home of a loved one, or in a long term care facility. Hospice also provides medical equipment such as a hospital bed, commode, and oxygen, along with supplies and prescribed medications for pain and symptom relief. It also covers outpatient treatments and short term hospital care, which may be needed when symptoms cannot be managed at home.

Our team will tailor a plan to meet your needs that will always respect and honor your wishes. Services include:

Attending physician: The patient has a right to choose an attending physician to oversee your care. This is usually your primary care doctor. You do not have to name one if you do not want to; and then our Medical Director will oversee your care.
Hospice Medical Director: Our Medical Director, Dr. Craig Concannon, oversees the nursing staff and hospice care.
Nursing care: Nurses will visit regularly, oversee your care and medications, provide care giving education, and communicate with your doctor. On call registered nurses are available 24 hours a day for questions and visits.
Hospice Aide: To assist with personal hygiene cares involving bathing, skin care, nail care, hair care, and oral care. They can also assist with light housework like sweeping, mopping, and dishes to ensure a clean comfortable environment.
Social Worker: To assess a patient's needs, situation, strengths, and support networks to determine their goals. To help patients adjust to changes and challenges in their lives and being the patient’s advocate. They can assist and provide counseling and connect you with additional resources when getting final affairs in order.
Spiritual support: To provide non-denominational spiritual support or assist in connecting you with a spiritual person of your faith. They can address spiritual needs, help find inner peace, improve quality of life and relieve spiritual distress. They will provide spiritual and emotional support to the patient and their family throughout the end of life journey.
Pharmacists Ensuring safe prescribing and dispensing of medications to patients. Monitoring for possible drug interactions and avoiding overutilization of medications. Addressing and managing symptoms to help ensure each patient receives quality end of life experiences.
Hospice Volunteers: Specially trained people to provide comfort, companionship, and support to patients and their families as they near the end of life.
Massage Therapy: Provides significant improvement for pain and anxiety while promoting a sense of peace, comfort and relaxation.
Dietary counseling: Available as needed
Physical and occupational therapy: Available as needed
Speech-language pathology services: Available as needed
Grief and Loss Counseling A companioning program designed to help a person work through the various stages and range of emotions one would feel after a loss. We guide the bereaved through their bereavement, helping to understand, accept, and manage emotions to live a full life.
Short-term Inpatient Care When symptoms or pain cannot be managed in the homecare setting a higher level of specialized care in an inpatient acute setting is provided until symptoms are controlled.
Continuous Care: Professional hospice services for terminally ill patients with uncontrolled symptoms. This care is provided to alleviate uncontrolled symptoms in a time of acute crisis until symptoms are controlled.
Respite Care Allows a family caregiver to get a break from the emotional and physically challenging role of providing end of life care for up to 5 consecutive days and nights while the patient is cared for in an inpatient facility.
Medical Equipment / Supplies Specialized equipment can be brought into your home to assist the family to be able to provide care to the patient. Available equipment includes hospital beds, air mattresses, lift chairs, tray tables, bedside commodes, wheelchairs, walkers, and concentrators. Supplies available include dressings, ostomy supplies, bedding, incontinence products and personal care products. We will work to get any other supplies and equipment necessary to provide care in the home.
Prescription Medications: Hospice will collaborate with your physician to obtain and maintain symptom control and pain relief with medications if necessary. Our team continuously reviews necessary medications as your needs shift to ensure comfort and enhance their quality of life.
